SUMMER MUSIC FESTIVAL 2009

Where: Attend this awesome weekend event at “The Grove” at All Saints Orthodox Church, 6004 South Fairfax Rd., Bloomington, IN 47401. All Saints sits south of the city on 24 mostly-wooded acres with quarry ponds and trails. Just an hour south of Indianapolis, Bloomington is home to the main campus of Indiana University. Who: All Orthodox teens are invited to come and bring their friends, as allowed by their chaperones.  Non-teens are welcome to attend as well, but only SOYO members and their guests and chaperones are invited to camp on the church grounds.  It is our hope and expectation that you will leave this event spiritually edified and uplifted by the music, prayer, teaching, games, and fellowship!

When: Arrival time begins at 9:30 a.m. on Saturday, June 13th.  Burgers, dogs, and bands begin around noon! Music, teaching, and events will continue through the late evening Saturday and follow the Divine Liturgy on Sunday.  MOSMoF ends at 4:00 p.m. Sunday. 

Cost of Admission: Each guest will pay $25.00 upon admission to the event. This   includes everything but food.

Food: Lunch and dinner (including gyros, grilled burgers, and hot dogs) will be available for purchase at the event for just $5.00/plate! Water is free; sodas are extra.

Lodging: Teens and chaperones may camp free-of-charge at designated sites on the All Saints property. Campers will be separated by gender. Please bring tents, sleeping bags, and pillows.  Bring bug repellant and a blanket to sit on during the concerts.  There are no showers. Attendees may choose to stay at a local hotel at their chaperone's discretion. 

Supervision: A chaperone of the same gender, over the age of 25, must accompany each group of 4 attendees. For example, a van carrying 4 teen boys and 2 teen girls requires one female and one male chaperone.  Chaperones should plan to sleep wherever their respective teens are sleeping.

Liability:  Teens must bring the completed release form (see website) for admission. Signature of a parent (or guardian) is required. 

Clothing: Attire should be consistent with Antiochian Village Camp requirements. Shoes and shoulder-covering shirts are required during all waking hours.  You won’t need muscle shirts, spaghetti-straps, tank tops, and skirts in the woods. Average temperatures range from 60-80 degrees this time of year.

A Word of Caution: Smoking, alcohol, drug use, and delinquent behaviors are prohibited and will result in attendees being sent home.

Permissions, Waiver, Release and Hold Harmless Agreement

In consideration of permission granted by All Saints Orthodox Church in Bloomington Indiana allowing me (or my child) to participate in the Midwest Orthodox Summer Music Festival (MOSMoF), which will occur June 13-14th  2009, which is sponsored in part by Midwest SOYO, I (together with my parent or guardian, if I am under the age of eighteen or under a legal disability) represent, covenant and agree, on behalf of myself and my heirs, assigns, and any other person claiming by, under or through me, as follows:

1.                    The person herein described has permission to engage in all event activities.

2.                    Healthcare:

 a.         Treatment: I hereby give permission to All Saints Orthodox Church and/or my chaperone to provide routine health care, administer over-the-counter medications, administer prescribed medications, and seek emergency medical treatment including ordering x-rays or routine tests. I agree to the release of any records necessary for treatment, referral, billing, or insurance purposes. In the event I (the parent/guardian) cannot be reached in an emergency, I hereby give permission to the individual selected by All Saints Orthodox Church to secure and administer treatment, including hospitalization, for the person named above.

b.         Transportation: I give permission to All Saints Orthodox Church to arrange necessary healthcare related transportation for me/my child. I also agree that if my child has to return home due to discipline violations, it will be at my own expense.

c.          Costs: I understand that my insurance coverage for my child will be used as primary coverage in the event medical intervention is needed. I further understand that I will be responsible for expenses not covered by my insurance.

d.         Precautions: I understand all reasonable safety precautions will be taken at all times by All Saints Orthodox Church and their agents during this event. I agree that my child will abide by all the rules and guidelines set forth by All Saints Orthodox Church for the safety and good health of the individuals at this event.

3.                    I acknowledge that participating in this event involves certain risks (some of which I may not fully appreciate) and that injuries, death, property damage or other harm could occur to me or others. I accept and voluntarily incur all risks of any injuries, damages, or harm which arise during or result from participation in this event, regardless of whether or not caused in whole or in part by the negligence or other fault of All Saints Orthodox Church, Midwest SOYO, The Antiochian Orthodox Christian Archdiocese, and/or its or their leaders, employees, and/or volunteers ("Released Parties").

4.                    I waive all claims against any of the Released Parties for any injuries, damages, losses or claims, whether known and unknown, which arise during or result from participation in this event, regardless of whether or not caused in whole or part by the negligence or other fault of any of the Released Parties. I release and forever discharge the Released Parties from all such claims.

 

5.                    I agree to indemnify and hold the Released Parties harmless from all losses, liabilities, damages, costs or expenses (including but not limited to reasonable attorneys' fees and other litigation costs and expenses) incurred by any of the Released Parties as a result of any claims or suits that I (or anyone claiming by, under or through me) may bring against any of the Released Parties to recover any losses, liabilities, costs, damages, or expenses which arise during or result from my participation in this event, regardless of whether or not caused in whole or part by the negligence or other fault of any of the Released Parties. 

 

6.                    I have carefully read and reviewed this Waiver, Release and Hold Harmless Agreement. I understand it fully and I execute it voluntarily.
